You must be gone for 2 consecutive weeks of classes in order to have your tuition prorated, and this form must be submitted PRIOR to your child’s absence.
All blanks must be filled in to have your tuition prorated. A confirmation email will be sent to your account as soon as the withdrawal has been processed.
